 Dead Space 2 adalah game survival horror yang terjadi di 2511, kira-kira tiga tahun setelah peristiwa Dead Space dan Dead Space: Buntut, pada sprawl, sebuah metropolis berpenduduk padat dibangun di atas beling dari Titan, bulan Saturnus. Penyakit mental yang disebabkan oleh Marker tampaknya bersifat permanen dan terus berpengaruh protagonis Ishak Clarke. Pemerintah akan terus memiliki tempat di storyline. Permainan akan dimulai di rumah sakit kapal sprawl dimana pemain akan menyaksikan kengerian kutu Necromorph seperti dimulai pada stasiun.
